We've made it easy for you to get started with our convenient digital new patient forms. Once you schedule your first appointment, we'll send your forms to you via email, so you can fill them out when it's convenient for you.
Unrestricted provider
At Fulbright Dental, we take a patient-first approach to dental care – our relationship is with our patients, not their insurance company. That's why we took on a Fee-For-Service model back in 2012, so we can offer the highest quality of care at a reasonable cost. That way, our patients can make decisions based on what's best for their health, not what their insurance dictates.
As such, we will work with all PPO insurance plans to help you maximize your benefits. Our knowledgeable team will assist you in navigating your coverage and understanding your options.
Here's how our insurance billing process works:
- Patients with a PPO insurance plan are charged up-front for the cost.
- We bill your dental plan at the time of service.
- If covered, the insurance company will then reimburse patients directly, less any co-pays and/or deductibles that may apply.

Young Child Essentials Annual Membership
- 2 professional dental cleanings
- Fluoride treatment
- Doctor exams (2 times per year)
- Needed x-rays (as determined by Dr.)
- 1 emergency exam per year (if needed)
- Laser cavity detection
- CariFree™ screening (cavity prevention)
- Up to 15% off all dental services (including cosmetic, surgical and implant procedures)
Essentials Annual Membership
- 3 professional dental cleanings
- Fluoride treatment

- 4 periodontal maintenance cleanings
- Fluoride treatment
- Doctor exams (2 times per year)
- Needed x-rays (as determined by Dr.)
- 1 emergency exam per year (if needed)
- Subgingival irrigation
- Oral cancer screening
- Laser cavity detection
- Laser gum therapy
- Digital photos
- CariFree™ screening (cavity prevention)
