What forms of payment does the FulCare Dental Membership Plan accept?

We accept debit and credit cards, including Visa®, Mastercard® and Discover®.

When can I start using my FulCare Dental Plan membership?

Once you enroll, your membership is effective immediately!

How should I schedule my preventive care appointments?

Scheduling your appointments is easy. Just call our office and set up an appointment for yourself and/or your family members. Our friendly staff will be happy to help.

How do I use my membership status for discounts on elective or cosmetic dental services?

When you call to arrange an appointment at our office, simply be sure to inform our staff that you are a member. Upon arrival, we will check that your membership status is “current” for a discount.

How do I know if I need the Adult Perio Gum Care plan?

Please be sure to call our office so our dentist can confirm that you are under perio maintenance. If you are a new patient, our dentist will suggest the plan most suitable for you after your initial exam.

I accidently missed my appointment. What should I do?

If you miss an appointment, you will be responsible for rescheduling. Our cancellation policy requires a two business day notice so you will need to check with our office to see if the $250 no show fee is required.

Is it okay to add members to my account?

Of course! We have plans for patients of all ages, and your entire family can even join.

Will I be able to transfer my membership to another dentist?

Because your FulCare Dental Membership Plan is specific to our practice, it is non-transferrable. If you happen to move to another area, we encourage you look for practices that offer alternatives to insurance coverage, as many dentists provide similar options.

If I cancel, will I receive a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

How much do I pay to sign up for the FulCare Dental Membership Plan?

Payment for joining the FulCare Dental Plan includes the $50 Lifetime Activation Fee in addition to your first month’s membership dues.

Can I prepay my FulCare Dental Membership Plan fees?

Yes we will offer you a 5% discount if you pay for your plan upfront.

What information is required for me to sign up for the FulCare Dental Membership Plan?

We will need your full name, date of birth, address, credit or debit card, and an email address. That’s it!
